There are a lot of really trippy horror films coming out abroad, particularly in Asia, many of which have been remade into American films (The Grudge, The Ring, Dark Water, The Eye, etc).
First, I'd recommend Three Extremes. It's a compilation of three Asian horror movies, roughly 30 minutes apiece. Each is different, each is tough to watch. One of the segments, "Dumplings," is also a feature-length horror film called Dumplings.
Another worth checking out is Audition. It starts off as an offbeat "looking for love" kind of movie, but it quickly evolves into something much darker and more disgusting. It's been years since I first saw it, but I still have a hard time sitting through it.
The Isle is a drama, but it also has two scenes involving fishing hooks that will make you cringe.
If I had to choose an American film that really shocked me, it would be Mysterious Skin. It's a complicated drama about sexual abuse. It's actually a great movie, but it's quite disturbing.
